What CTEM Actually Is

The term Continuous Threat Exposure Management is sometimes used interchangeably with vulnerability management, attack surface monitoring, or risk-based patching. It is none of these — or rather, it is the operational model that connects and elevates all of them.

CTEM is a continuous cycle structured around five stages: scoping, discovery, prioritisation, validation, and mobilisation. Each stage builds on the last, and the cycle repeats without a fixed end point. The key word is “continuous” in the operational sense: exposure state is tracked over time, remediation is verified rather than assumed, and the programme evolves as the environment changes.

What distinguishes CTEM from vulnerability management is not primarily a question of frequency. It is a question of design. Vulnerability management programmes were built to answer “what is broken?” CTEM is built to answer “what is exploitable, what does it connect to, and can we prove it has been addressed?” That reorientation changes almost everything downstream: how assets are scoped, how findings are prioritised, how remediation is validated, and how outcomes are reported to the business.

The framework also makes an explicit commitment that most security programmes avoid: it treats the attack surface as a dynamic environment rather than a static inventory. New assets are deployed. Configurations drift. Dependencies shift. A CTEM programme is designed to account for that movement rather than pretend it does not happen between scans.

The Structural Difference Between Periodic Assessment and Continuous Validation

Annual penetration tests and quarterly vulnerability scans remain valuable. They are not, however, sufficient as a primary posture assurance mechanism in environments that change daily.

The maths are stark. Research published by Mandiant in 2024 found that attackers weaponise vulnerabilities within an average of 15 days of disclosure. (Mandiant, M-Trends 2024) The average enterprise remediation time, per Qualys’ 2024 TruRisk Report, sits between 60 and 90 days. (Qualys, TruRisk Research Report 2024) By late 2024, Google’s Project Zero found the average time-to-exploitation after disclosure had compressed to five days for high-profile vulnerabilities. A quarterly scan captures a moment. By the time the report is written, reviewed, prioritised, and acted upon, the threat environment has moved on.

The problem compounds when organisations look at what happens after tickets are closed. Research analysing remediation activity across tens of thousands of vulnerable systems found that most sectors remediated only 30 to 45 per cent of vulnerable instances within 150 to 200 days — and that is before accounting for whether those patches actually held. (Bitsight, Patch vs. Workaround: How CVEs Actually Get Fixed, 2024) The scale and pace of exploitation is accelerating: VulnCheck found that 28.3% of exploited vulnerabilities were weaponised within a single day of CVE disclosure in Q1 2025, while Mandiant’s M-Trends 2025 report confirmed that vulnerability exploitation remained the most common initial infection vector for the fifth consecutive year, accounting for one in three attacks across 2024. (VulnCheck, Q1 2025 KEV Report; Mandiant, M-Trends 2025)

The point is not that organisations are negligent. It is that the operational model of periodic assessment creates a structural gap that determined adversaries are designed to exploit. A penetration test conducted in October does not tell you whether the configuration change deployed in November opened a new attack path. Continuous validation does.

What continuous validation looks like in practice involves automated testing running in parallel with production environments, exposure state tracked over time (not captured at a moment), adversarial simulation used to confirm that controls perform as expected under realistic conditions, and remediation verified against a defined exposure baseline rather than a ticket status. It is a fundamentally different operating rhythm — and one that is increasingly necessary given the pace at which enterprise environments change.

Scoping and Prioritisation: Where CTEM Starts

A CTEM programme does not begin with a scan. It begins with a scoping exercise that answers a harder question: of everything in our environment, what matters most to the business?

This business-context-first approach is the most important distinction between CTEM and volume-based vulnerability management. Organisations with mature vulnerability programmes already know they cannot patch everything. Research from XM Cyber found that in large enterprises, approximately 75% of exposures do not lead to another asset — they are dead ends to an attacker. Only around 2% of exposures actually lead to critical business systems. (XM Cyber, CTEM Overview and Research, 2024) The CTEM scoping stage exists to identify that 2% before the rest of the programme begins, so that discovery, prioritisation, and validation efforts are oriented around the outcomes that matter.

Prioritisation within CTEM is governed by three intersecting factors: the exploitability of a given exposure (is there a working exploit? Is it being actively used?), the criticality of the asset in question (what business processes depend on it? What regulatory obligations apply?), and the business consequence of compromise (operational disruption, data loss, regulatory penalty, reputational damage). CVSS scores remain useful as one input, but organisations that rely on them as the primary filter for remediation priority are answering the wrong question. A CVSS 9.8 vulnerability on a non-internet-facing, isolated test system may represent lower genuine business risk than a CVSS 6.5 vulnerability on a system handling financial transactions or safety-critical operational technology.

This is not a theoretical argument. The ASD’s ACSC Annual Cyber Threat Report 2024–25 reported that critical infrastructure entities were notified of malicious activity more than 190 times during the financial year — a 111% increase. (ASD’s ACSC, Annual Cyber Threat Report 2024–25, 2025) For organisations operating in utilities, resources, and other critical sectors, the consequence model for exposure is not primarily financial. It is operational. CTEM’s business-context-first scoping approach is not a conceptual preference; it is an operational necessity.

Validation: The Step Most Programmes Skip

Detection and remediation are the visible parts of the security operations cycle. Most programmes invest heavily in both. Validation — confirming that remediation actually closed the exposure — is the step that most programmes skip, and it is the step that determines whether the work done in the rest of the cycle translates into genuine resilience.

The consequences of skipping validation are concrete. A patch applied but not properly deployed. A configuration change that closes one attack path while inadvertently opening another. A firewall rule that is set correctly but interacts with a downstream dependency in a way that preserves exploitability. These scenarios are not edge cases. The 2024 Verizon Data Breach Investigations Report found that a majority of vulnerabilities in CISA’s Known Exploited Vulnerability (KEV) catalogue remained unresolved 60 days after being added — meaning organisations had been notified, had presumably assigned remediation tasks, and still had not actually closed the exposure. (Verizon, Data Breach Investigations Report, 2024) The remediation ticket was resolved; the exposure was not.

CTEM’s validation stage addresses this directly. Using adversarial testing, breach and attack simulation, or targeted manual verification, validation confirms that the exposure no longer exists in the environment — not just that the remediation activity was completed. That distinction is the difference between measuring activity and measuring resilience.

That is a fundamentally different posture conversation — and one that holds up under scrutiny.

What a CTEM Programme Looks Like in Practice

Understanding the concept of CTEM is straightforward. Implementing it requires a clear view of what the operating model actually looks like: governance, integration, cadence, and measurement.

Governance starts with ownership. A CTEM programme requires a programme owner with cross-functional authority — someone who can engage both security operations and the business units whose asset criticality and risk tolerance shape scoping decisions. Without that structural connection, the programme defaults to a technical exercise disconnected from the business risk framework it is supposed to serve.

Cadence is continuous in principle but structured in practice. Most mature CTEM programmes run automated discovery and monitoring continuously, conduct structured validation cycles on a defined rhythm (monthly or quarterly, depending on environment complexity and risk profile), and report outcomes against defined exposure reduction metrics rather than against activity volume. The board-level question — “is our exposure reducing over time?” — needs a measurable, credible answer, and the programme’s cadence should be designed to produce one.

Integration with existing security operations is critical and often misunderstood. CTEM does not replace a SOC, a vulnerability management programme, or a penetration testing schedule. It provides the connective framework that makes those investments more effective. Discovery feeds from existing asset inventory and monitoring tools. Prioritisation uses threat intelligence already flowing through the security stack. Validation can use existing red team or BAS capabilities. Mobilisation connects to existing ITSM and remediation workflows. The programme adds operational design and discipline; it does not require wholesale replacement of what is already in place.

The first 90 days of a CTEM programme typically focus on three things: establishing the scope and asset criticality model with business input, conducting an initial discovery cycle to baseline current exposure state, and running a first validation exercise to identify the gap between assumed and actual remediation effectiveness. That gap — consistently larger than organisations expect — usually provides the business case for the continuous programme.

Outcome measurement in a mature CTEM programme moves away from traditional vulnerability management metrics (total vulnerabilities found, total tickets closed) toward exposure-based indicators: reduction in business-critical exposures over time, reduction in attack paths leading to priority assets, mean time to validated remediation (not just mean time to ticket closure), and percentage of remediation verified against the exposure baseline. These metrics connect technical security work to the business risk outcomes that executives and boards need to assess.

Artificial intelligence is increasingly present in CTEM discussions, and for legitimate reasons. AI adds measurable value in several specific parts of the CTEM cycle: correlating threat intelligence at scale to assess exploitability in context, identifying attack path patterns across complex hybrid environments, accelerating discovery by processing telemetry volumes that exceed manual analysis capacity, and surfacing prioritisation signals that human analysts might otherwise miss.

The evidence base for AI’s contribution to security operations is substantial. IBM’s Cost of a Data Breach Report 2024 found that organisations using AI and automation extensively identified and contained breaches approximately 98 days faster than those that did not, with average breach costs USD 1.88 million lower. (IBM, Cost of a Data Breach Report 2024) The 2025 edition found the mean time to identify and contain a breach had fallen to 241 days — the lowest in nine years, driven in part by wider AI adoption in security operations. (IBM, Cost of a Data Breach Report 2025)

These are meaningful outcomes. But they are outcomes achieved by organisations that had the foundational programme elements in place: reliable asset inventory, mature telemetry, validated remediation processes, and clear escalation paths. AI in those environments accelerated work that was already being done systematically. In environments where the foundations were absent — where asset visibility was incomplete, telemetry integrity was inconsistent, or remediation verification did not exist — AI amplified those gaps rather than compensating for them.

The implication for CTEM is clear. AI should be introduced into a CTEM programme where it accelerates the specific stages that benefit from it: discovery correlation, prioritisation signal processing, and attack path analysis. It should not be positioned as a substitute for the operational design decisions — scoping, governance, validation, mobilisation — that give those analytical outputs something meaningful to work on. Organisations that begin their CTEM journey by asking “which AI tools should we deploy?” have started at the wrong end of the problem.
